Sports Registration
Marysville School District Athletic Department has partnered with Final Forms, an online forms and data management service. Final Forms allows you to complete and sign athletic participation forms for your students. The most exciting news is that Final Forms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! Final Forms also pre-populates information wherever possible, for each of your students, saving you time.
Parents and guardians will be able to review the data at any time to verify it is current and are required to sign the online forms each school year and after any update.
How Parents Access Final Forms.
IMPORTANT: Once a student is registered and active in Skyward, the Skyward program uploads that information to Final Forms every night at midnight. Parents and guardians will receive an account confirmation email from Final Forms . When you receive the email, please click the link in the email to confirm your account and then create a password. Once you are logged into Final Forms you will see all of your students listed. To the right of each student is a +Add Sports button. Click on that button to add the sports that your student would like to participate in. Once you select the sport(s) then all of the forms are created for the parent to fill out, sign and submit. Once the parent signs and submits all of their forms then your student will receive an email in their MSD student email account from Final Forms . They will need to click on the link and create a password for their Final Forms student account so they can sign and submit the student forms. Students will only have forms to sign once a sport is selected. All students are required to have a current Sports Physical in order to play sports.
You only need to do this if your child(ren) plans on participating in a sport. If you don’t receive an account confirmation email please contact liz_doyle@msd25.org or call her at (360)965-0111 because that means we don’t have a parent email listed in your child's Skyward account. A parent email is required as that is what is used to create the parent Final Forms account.
